Confidential individual therapy sessions are offered for people ages 18 and up. Individual therapy is aimed at helping people reach their identified goal(s) for change, and is tailored to one’s specific unique needs. Sessions are typically 50 minutes in length and scheduled on a varied or recurrent basis. Letters of support for gender -affirming medical treatment can be done as part of therapy. People may use insurance benefits for sessions, or pay the self pay rate of $150 per session.
Sarah is currently in network with Husky, Cigna, and Aetna insurances.
For all other insurances, Sarah works with Mentaya, a platform that helps clients get money back on out-of-network therapy sessions. If you have out-of-network benefits, Mentaya will file claims and handle the insurance paperwork to make sure you get reimbursed. They charge a 5% fee per claim and have helped people get thousands of dollars back per year. You can sign up using this link:
Note: Mentaya’s goal is to save you time and money. It’s completely optional, and as your therapist I do not benefit in any way from your participation.
Letters of support for gender-affirming medical treatment:
Transitions Therapy LLC provides assessments for gender-affirming surgical procedures. As a seasoned, trans competent therapist, Sarah Gilbert, LCSW has great familiarity with the process of writing letters of support for cross-hormone treatment and surgical procedures as well as the WPATH Standards of Care. This is offered as a stand-alone service; meaning that you can schedule sessions with Sarah just for an assessment, rather than ongoing therapy. As someone who’s committed to not be a gatekeeper in trans healthcare, Sarah does not require a set number of sessions before this letter will be completed. People may use insurance benefits for sessions, or pay the self pay rate of $150 per session. Additionally, Sarah Gilbert has signed the GALAP pledge, and provides one free session per month for those seeking an assessment for gender-affirming healthcare.
For licensed mental health providers who need one-time or ongoing consultation regarding their work with trans-identified clients, Transitions Therapy LLC offers individual and group clinical consultation. Investment: $160 per one hour session, $80 for a 30 minute session.
Each month, Sarah also facilitates a virtual clinical consultation group for trans-affirming providers; it’s held on the first Tuesday of each month from 9am-10:30am EST. You can register for this on the events page of this site, or by emailing [email protected]

Reviews from past training participants:
“Thank you so much. Training was awesome and I appreciated the centering of lived trans experiences through videos, sharing of books, etc. Also, the resources list is phenomenal!”
“It was really cool that Sarah started the training acknowledging that she is cis and not walking around the world as a trans person and how that has an impact on her knowledge base. I have never heard someone do that and as a non-binary person, it was much appreciated.”
“Most continuing education says you will leave with skills you can immediately use but most often fall short. This training said there would be immediate take away skills and ideas and it was definitely true!!’
‘Awesome training! I was sharing information with my families the same day.”
“Sarah was a great presenter and is clearly very knowledgeable. She created an environment for attendees to feel safe and vulnerable to ask difficult questions. I feel much more knowledgeable and prepared to better serve this community. Thank you!”
“Fantastic training, and thank you for having such an affirming environment. Sometimes therapist spaces don’t do that, so it was nice to be a little vulnerable and make actual connections.’
“Loved this training, trainer was super engaging, which is difficult in an 8 hour virtual training!”
“This training was very informative!!!!!! I loved how she brought out cis bias.”
