Individual therapy:

Confidential individual therapy sessions are offered for people ages 18 and up. Individual therapy is aimed at helping people reach their identified goal(s) for change, and is tailored to one’s specific unique needs. Sessions are typically 50 minutes in length and scheduled on a varied or recurrent basis.  Letters of support for gender – affirming medical treatment can be done as part of therapy. People may use insurance benefits for sessions, or pay the self pay rate of $160 per session.

Sarah is currently in network with Husky, Cigna, and Aetna insurances.

Transitions Therapy LLC is primarily a telehealth practice, with limited in-person appointment availability in East Harford, CT.

Letters of support for gender-affirming medical treatment:

Transitions Therapy LLC provides assessments for gender-affirming healthcare.  As a seasoned, trans competent therapist, Sarah Gilbert, LCSW has great familiarity with the process of writing letters of support for cross-hormone treatment and surgical procedures as well as the WPATH Standards of Care.  This is offered as a stand-alone service; meaning that you can schedule sessions with Sarah just for an assessment, rather than ongoing therapy.  As someone who’s committed to not be a gatekeeper in trans healthcare, Sarah does not require a set number of sessions before this letter will be completed. People may use insurance benefits for sessions, or pay the self pay rate of $160 per session.

Clinical Consultation:

For licensed mental health providers who need one-time or ongoing consultation regarding their work with trans-identified clients, Transitions Therapy LLC offers individual and group clinical consultation.  Investment: $160 per one hour session, $80 for a 30 minute session.

Trainings, speaking on trans-affirming care:

Transitions Therapy LLC is committed to expanding the capacity for greater trans inclusivity in professional, educational, and treatment environments. Since 2018, Sarah Gilbert has trained hundreds of mental health professionals, and has also delivered trainings at various agencies and schools in CT. Out of this commitment, Sarah creates customized trainings on trans-affirming and inclusive care; delivered virtually or in person if in CT.

Starting in 2025, Sarah will be providing these trainings exclusively to schools, healthcare facilities, agencies and organizations. If you would like Sarah to provide a training for your staff, please send a training/speaking request to [email protected]. In the request, please include desired training topic, your budget, and estimated number of attendees.
